•Employees will be provided with a written lockout process when required to work on normally energized equipment.
•Where the lock out process uses a lock and key, a lock will be issued that is operable by the worker’s key and duplicate key.
•In the event a different Power Tech employee must finish a job where lock out's have been applied by a previous Power Tech employee, a duplicate key will be available to be used, as long as that employee is finishing the job and removing the lock.
•Where the lock out process does not use a lock and key, Power Tech management will coordinate and control the lock out process.
•Prior to undertaking any maintenance or repairs or adjustments to an ordinarily energized piece of equipment the written lock out process will be followed.
•The employee who initiated the lockout process will fill out the Power Tech lockout form to ensure all steps have been followed. Once the work has been completed, the employee will remove the lock and fill in the required fields in the form to ensure all post lockout steps have been followed. The form shall be returned to the main Power Tech office.
•After the lock out process has been initiated or a lock installed, the worker who initiated the process or installed the lock shall test to ensure the machine is inoperative.
•Only keyed locks will be used for lock out. “Do not operate” tags alone do not satisfy the requirements of the OH&S regulations.
