Lock Out Procedure

Perform all work as per Canadian Electrical Code, OH&S Regulations, & company policies.

Ensure proper PPE (steel toed boots, coveralls, safety glasses, hard hat, H2S monitor)

Disconnect & tag field switch (if applicable)

Disconnect device main switch

Test output voltage to device at main switch & at device & ensure voltage has been disconnected (zero voltage).

Ensure lock has tag with name, phone #, company name, & short job description.  Fill out the Power Tech lockout form to ensure all steps have been followed.

Install lock.

Only person who installed lock & tag can remove it or duplicate key procedure must be followed.

Before any work begins on an electrical conductor or electrical equipment and during the progress of that work, ensure that the electrical conductor or electrical equipment is isolated, locked out and connected to ground; or other effective procedures are taken to ensure the safety of the workers.

Once the work has been completed, the employee will remove the lock and fill in the required fields in the form to ensure all post lockout steps have been followed.

The form shall be returned to the main Power Tech office.
