All employees and contractors are responsible for:
•Ensuring that all injuries, near misses, unsafe conditions, unsafe work practices, illness, accidents, property damage, time loss incidents, first aid incidents, medical aid incidents, and work refusals are verbally reported immediately.
•Completing a preliminary incident report giving a detailed description of the loss or near loss, including near misses, unsafe conditions, unsafe work practices, illness, accidents, property damage, time loss incidents, first aid incidents, medical aid incidents, and work refusals incidents.
•Ensuring the applicable report is given to Management before the end of the next working day. This information is the key to preventing further occurrences.
Management is responsible for:
•Ensuring that all injuries, near misses, unsafe conditions, unsafe work practices, illness, accidents, property damage, time loss incidents, first aid incidents, medical aid incidents, and work refusals have a written report including photographs if available and to determine the cause & implement immediate corrective action and document any long term action to prevent re-occurrence.
•Determining whether a full investigation is required.
•Forwarding copies of all reports to the Estevan office as required by these procedures.
•Coordinating and ensuring an investigation is completed immediately if an investigation is required.
•Ensuring remedial action has been completed by the specified date.
•Reviewing all reports to ensure correctness and completeness.
•Completion of the management control section, assigning responsibility for remedial action and follow-up.
