All injuries, near misses, unsafe conditions, unsafe work practices, illness, accidents, property damage, time loss incidents, first aid incidents, medical aid incidents, and work refusals must be reported on a Power Tech Industries Ltd. incident report form on or before the next working day. Any employee or contractor involved in the incident may be required to complete a report form and assist in the investigation, if an investigation is deemed necessary.
"Incident" is defined as "an undesired event which results in, or could have resulted in, harm to people, damage to property or loss to process." This definition concentrates on harm, damage and loss and may be further expanded to include:
•Personal injury requiring medical aid (where professional medical personnel or facilities are required);
•Damage to property such as vehicle, fire, explosion, environmental, security, etc. related incidents where such damage resulted;
•Loss of equipment and materials.
“Near Miss” is defined as an unplanned event that did not result in injury, illness, or damage – but had the potential to do so. Only a fortunate break in the chain of events prevented an injury, fatality or damage.
